Creating service requests on the service request portal
To create a service request on the service request portal:
- Click the URL provided to open the service request portal. The Service Request Portal page is displayed.
- Click Create Request. The Create Service Request page is displayed.

Specify this Location Details information:
- Equipment
- Specify the name of the equipment needing service.
- Campus
- Specify the name of the campus needing service.
- Building
- Specify the name of the building needing service.
- Floor
- Specify the name of the floor needing service.
- Room/Unit
- Specify the room or the unit needing service.

Specify this Request Details information:
- Service Category
- Specify the category identifying the requested service.
- Service Code
- Specify the code identifying the requested service.
- Request Details
- Add any details about the service you are requesting or any helpful information.
- Type
- Specify the request type for the service.
- Priority
- Specify the priority of the service request.
- Department
- Specify the department responsible for the requested service.
- Supplier
- Specify the supplier for the equipment on this service request.
- In the Contact Details section, specify the First Name, Last Name, E-mail, and Phone of the person at the site needing service, or the designated person to contact.
- Optionally, select the Request Registration check box if you are a guest user and would like to register, and use your e-mail address as your Portal ID for future log-in access.

Optionally, click the Document Upload button to upload and attach
a document to the service request.
Note: You must be an Infor Ming.le™ user for access to the document upload feature. Additionally, the CCMNGLID parameter must be populated with a valid EAM user ID. Contact your system administrator for more information.

Click Submit.
Note: If you are a guest user and selected the Request Registration check box, the service request is saved and you are logged out. Re-enter the portal with the e-mail address you specified as your Portal ID.
- Optionally, to create more than one service request, click Submit and Add Another. The first service request is submitted, and the screen returns to insert mode.
